Kanniewagnie Posted July 30, 2016 Report Share Posted July 30, 2016 We used vacation care last school holiday and centrelink advised us that the children's immunisation is not up to date. We have the immunisation records from South Africa. There is a form to complete on the centrelink website - but the names of the vaccines are different and the age when administered as well. A Dr or other qualified person must complete & sign the form which must be submitted to update that records on their system before we can try to claim again. Where did you guys go to do this form - will I be able to do it in one appointment at the Dr for both children, or where else can I go for this? Thanks.
