SDTCPT Posted January 8, 2015 Report Share Posted January 8, 2015 Hi there,Just my 2 cents here.Just be careful with printing these from the web. On the DHA website it says that only original forms will be accepted. Went to apply for mine on the 20th of Dec last year. The forms for the birth certificates I got from the Malmesbury DHA office had 2 or three "carbon" copies attached and when I went to the Paarl DHA office they gave me two extra forms to complete for the birth certificate along with the original forms I got from Malmesbury.
