The South African one looks like this:

DECLARATION BY APPLICANT

I, full names and surname, South African ID number: xxxxxxxxxxxxxxxxxx, South African Passport number: xxxxxxxxxx at present residing at .........................................

Do hereby solemnly and sincerely declare that:

I AM NOT IN AUSTRALIA AT THE TIME OF LODGING MY APPLICATION. (or whatever you are declaring)

DECLARATION BY COMMISSIONER OF OATHS

I certify that before administering the solemn declaration I asked the deponent the following questions and wrote down the answers in his presence:
