Ruizinho Posted May 16, 2017 Report Share Posted May 16, 2017 I really need clarity. I am applying now for the 189 visa. For my wife I need to add her as a family member on my visa. Her name has changed we have the name change proof The visa application asks for her name surname and passport number. Her passport however reflects her maiden name not her new name but the passport is still valid. I applied under my maiden name as was in my passport, even though my surname was changed (my husband was main applicant)..
